Your personalized organ donor card displays your personal details as well as the details of your next of kin.

Please follow the steps carefully to ensure your organ donor card arrives at your postal/residential address.

2. On the next page enter your email address and press "Checkout”
3. Thereafter enter your shipping details and press "Continue”
4. The next page will display your email and delivery address and confirm free shipping. Press "Continue" and thereafter press "Go to Payment”
5. Complete the payment process.

Once the payment process is complete, you will receive an email containing a link that takes you to the page where you will fill in your contact information and the name and number of your next of kin. Please make sure you follow the directions carefully on the page dedicated to capturing your personalized organ donor card.
